Contact and Hospitalization Form link. This form is used to notify residents that you are in the hospital, Please fill it out, print and sign it and return to Resident Life. You can place it in the Resident Life folder at the front desk.

Pastoral Ministries helps residents to celebrate life by sharing each others wisdom, diversity and spirituality! Therefore, it is the residents of Ann’s Choice who cultivate all the Pastoral Ministries activities which occur here.
Regardless of your faith affiliation, you are always invited to attend all functions of Pastoral Ministries and Ann’s Choice Faith Councils. It is a wonderful way to learn about different faiths and to meet and create new friendships within the Ann’s Choice community.
Each Faith Group has a handbook which is available in the foyer of the Chapel or in the Resident Life Suite. These handbooks will explain each individual faith council in more depth. Please contact Pastoral Ministries with any questions @ 215-443-3831.
Please note: There are times when cancellation is necessary for some events that are scheduled and is beyond the control of Pastoral Ministries and Ann’s Choice Faith Groups.
